What do I need?
1) Application by the person concerned, if he/she is an adult or by the acknowledged person or the mother of the child, if he/she is a minor.
2) A notarial deed of voluntary recognition or, if the recognition is judicial, a copy of the Judicial Recognition Decision and a certificate from the Registry of the Court of First Instance and a report of service or a report of waiver of legal remedies.
3) Birth Certificate with the change in the margin (registered recognition in which the surname determination is mentioned and the child is a citizen)
4) Certificate of the acknowledged father from the Boys’ Registers.
5) In case of marriage of the parents, a marriage certificate and a certificate of marital status.
