What do I need?
1) Application of the person concerned, if he/she is an adult, or his/her father or mother if he/she is a minor.
2) Corrected birth certificate.
3) Certificate of registration in the Register of Boys of the father or registration in the Municipal Register of the mother respectively (when they are foreigners, a birth certificate from their country of origin is required, which must be officially certified and translated).
4) Certificate of marital status (in case the father or mother is a foreigner, they must appear in an indicative record with the correct main name respectively).
