What do I need?
1) Application
2) A copy of the adoption court decision, a certificate of the Registry of the Court of First Instance and a report of service
3) Birth Certificate (photocopy of the page of the civil registry office with the new data entered)
4) Extract from the Birth – Baptismal Register (original) before the new birth certificate is drawn up
5) Certificate of registration in the Boys’ Registry of the adoptive father
6) Certificate of registration in the Municipal Register of the adoptive mother or certificate of marital status (of the Municipality or Community to which the adoptive parents belong)
7) Marriage certificate of the adoptive parents
