What do I need?
1) Application of the person concerned, if he/she is an adult, or his/her father or mother if he/she is a minor
2) Birth certificate from the competent Registry Office for those born in Greece. For those born outside Greece, a birth certificate is required ONLY from the Special Registry of Athens with the correction registered.
