What do I need?
1) Application (not required for online submission)
2) Certificate of registration in the Register of Boys of the father or registration in the Municipal Register of the mother respectively (when they are foreigners, a birth certificate from their country of origin is required which must be officially translated).
3)Corrected birth certificate.
4) Certificate of marital status (in case the father or mother is a foreigner, they must appear in an indicative entry with the correct main name respectively).
